Abstract The Japanese occupation of Malaysia during the Second World War has occupied a significant space in national history textbooks in Malaysia. The period has been associated with nationalist movements and independence from colonial rule. However, narratives of the Japanese occupation in school history textbooks have changed in terms of the importance given to the experiences of the three major ethnic groups in Malaysia (the Malay, Chinese, and Indian ethnic groups). This article presents an analysis of the Japanese occupation as portrayed in Malaysian national history textbooks from 1978 to 2020. It demonstrates a link between ideology and the state curriculum in Malaysia, which shows how cultural and ideological factors (namely, the political ideology of Malay dominance, or ketuanan Melayu) explain the changes made to the narratives in history textbooks.

Use of Galactagogues in a Multi-Ethnic Community in Southeast Asia: A Descriptive Study.

PurposeGalactagogues are substances that increase breast-milk production. They can be medication, herbs or food. Use of galactagogues may be different among the major ethnic groups in Malaysia. The primary objective of this cross-sectional study is to determine the prevalence of galactagogues use among breastfeeding mothers in a multi-ethnic community.Patients and MethodsSelf-administered questionnaires were distributed between November 2016 and January 2017 to mothers attending the health clinics and private hospital paediatric clinics in Penang, Malaysia, whose infants were below 6 months of age. The questionnaire enquired about use of galactagogues, types of galactagogue used, perceived effectiveness and sources of information. It also asked mothers about what they would do to increase milk supply.ResultsWe analyzed 322 out of 530 questionnaires distributed where 76% of mothers reported use of galactagogues; most of which were food-based: dates (66%), oats (58%) and soya bean (56%). Each major ethnic group had a different preference for galactagogues: Malays (dates), Chinese (soya bean) and Indians (oats). Only one mother used pharmacological galactagogues alone. Galactagogues were perceived to be effective by 65% of mothers who used them. Multivariate analysis showed that use of galactagogues was not related to ethnicity, education, work, perception of milk supply or use of formula milk. The most common source of information on galactagogues came from family and friends.ConclusionUse of galactagogues is common in our population, especially food-based ones, and the choice of galactagogues differs by ethnicity. Future studies on efficacy and safety galactagogues should be focused these commonly used ones. There is also a need for qualitative studies to improve our understanding of why they are widely used in this region.

When Health Intersects with Gender and Sexual Diversity: Medical Students’ Attitudes towards LGBTQ Patients

ABSTRACT A central tenet of the health professions is that of equitable access to health care. However, disparities in equitable healthcare provision continues to be a challenge in many societies due to prejudices against the LGBTQ community. This study was aimed at exploring the attitudes of medical students toward LGBTQ patients in Malaysia. A qualitative approach was adopted to seek depth of understanding of clinical year medical students’ perceptions and attitudes toward LGBTQ patients. Data were collected in 2018 through individual interviews and focus group discussions with a total of 29 participants, using a semi-structured question guideline. Purposive sampling comprised representation from the three major ethnic groups in Malaysia. Thematic analysis using NVivo highlighted three main themes i.e., neutrality, in compliance with the Professional Code of Conduct; implicit biases and tolerance of an Odd Identity; explicit biases with prejudices and stereotyping. The lack of knowledge and understanding of the nature and issues of sexuality is problematic as found in this study. They are primarily biases and prejudices projected onto marginalized LGBTQ patients who must contend with multiple jeopardies in conservative societies such as in Malaysia. With some state policies framed around Islam the concern is with the belief among Malay/Islamic students for LGBTQ individuals to go through conversion ‘therapies’ to become cisgender and heterosexual.

ISLAMIC INFLUENCE ON WOODCARVING MOTIFS IN JOHOR TRADITIONAL MALAY HOUSES

Woodcarving motifs can be influenced by various aspects, including religions and beliefs. Malaysia is known for its cultural diversity and is rich with traditional heritage. Malay is one of the major ethnic groups in Malaysia and is considered as the majority of the country's population, contributing to the significant heritage, including art, decoration, and even architecture. Traditional woodcarving is an intricate Malay heritage and is the symbol of Malay art. A study on Malay woodcarvings is critically needed in order to know the influence that occurs on the selection of the motifs. It is such a waste to see our woodcarving heritage destroyed unappreciated and undocumented, especially on Johor woodcarvings, as many studies in other states have done. More and more traditional Johor Malay houses are being demolished day by day before we know the historical influence on the existence. The objective of this paper was to identify the woodcarving motifs found in Johor Malay houses and to identify their influence. Site visits were conducted in which the woodcarvings of 50 houses were examined, photographed, and documented. Interviews were also conducted with the house owners and the woodcarvers. The samples were analyzed, and the results were verified by experts. The results show that floral motifs lead the statistic, and the pumpkin motif, known as 'labu maya', takes the highest percentage of the overall motifs. It is concluded that the Islamic religion is the biggest influence that contributes to the findings. The findings also show that the 'labu maya' motif is the identity of the Johor woodcarving motifs. It is hoped that this study can bring recognition to Johor woodcarvings, and contribute valuable guides for woodcarvers to apply these motif typologies into their future works to sustain and cherish the Johor local identity.

Geographical separation and ethnic origin influence the human gut microbial composition: a meta-analysis from a Malaysian perspective.

Ethnicity is consistently reported as a strong determinant of human gut microbiota. However, the bulk of these studies are from Western countries, where microbiota variations are mainly driven by relatively recent migration events. Malaysia is a multicultural society, but differences in gut microbiota persist across ethnicities. We hypothesized that migrant ethnic groups continue to share fundamental gut traits with the population in the country of origin due to shared cultural practices despite subsequent geographical separation. To test this hypothesis, the 16S rRNA gene amplicons from 16 studies comprising three major ethnic groups in Malaysia were analysed, covering 636 Chinese, 248 Indian and 123 Malay individuals from four countries (China, India, Indonesia and Malaysia). A confounder-adjusted permutational multivariate analysis of variance (PERMANOVA) detected a significant association between ethnicity and the gut microbiota (PERMANOVA R 2=0.005, pseudo-F=2.643, P=0.001). A sparse partial least squares – discriminant analysis model trained using the gut microbiota of individuals from China, India and Indonesia (representation of Chinese, Indian and Malay ethnic group, respectively) showed a better-than-random performance in classifying Malaysian of Chinese descent, although the performance for Indian and Malay were modest (true prediction rate, Chinese=0.60, Indian=0.49, Malay=0.44). Separately, differential abundance analysis singled out Ligilactobacillus as being elevated in Indians. We postulate that despite the strong influence of geographical factors on the gut microbiota, cultural similarity due to a shared ethnic origin drives the presence of a shared gut microbiota composition. The interplay of these factors will likely depend on the circumstances of particular groups of migrants.

A Forensic Study of Ethnicity and Sex Differences in Fingerprint Patterns in a Malaysian Subpopulation

Fingerprint is one of the most common evidence found at a crime scene. Owing to the uniqueness of fingerprint, it is useful for suspect identification. Typically, the questioned fingerprint is of limited contribution due to blurriness of the minutia or absence of potential suspect. However, it is still useful if the ethnicity or sex of the questioned fingerprint could be inferred to narrow down the scope of searching of suspect. Therefore, this study aims to examine sexual and ethnic differences in fingerprint patterns among the three major ethnic groups in Malaysia, i.e. Malays, Chinese and Indians. In total, 2000 plain fingerprints were collected from all the ten fingers of 200 Malaysians (i.e. 99 males and 101 females). Each of the three ethnic groups was represented by at least 60 subjects. Descriptive statistics and hypothesis test were performed to assess the sexual and ethnic variability of fingerprint patterns. In addition, simple correspondence analysis was also conducted to support the descriptive and inferential statistics. By considering fingerprint patterns in all the ten fingers, loop was the most frequent pattern in Malays (23.0%) and Indians (23.2%), while Chinese (13.9%) showed slightly higher frequency of whorl than Malays (9.6%) and Indians (10.3%). The relative distribution of fingerprint patterns of male was similar to that of female. In conclusion, fingerprint patterns could be used to infer the ethnicity rather than the sex of a suspect in the Malaysian context.

Clinical and demographic pattern of chronic granulomatous disease (CGD) from a multicenter perspective: Malaysia\u2019s experience over 26 years

BackgroundA retrospective review of clinical manifestations and demographic pattern of patients diagnosed as chronic granulomatous disease (CGD) from 7 hospitals in Malaysia. An analysis of the available database would establish clinical characteristics, diagnoses and outcome including microbiologic pattern. Studying the demography allows us to document the occurrence of CGD amongst multiethnic groups and its geographical distribution for Malaysia.MethodsData from the Malaysia Primary Immunodeficiency Network (MyPIN) with cases of CGD diagnosed from 1991 until 2016 were collated and analysed.ResultsTwenty patients were diagnosed as CGD. Males (N = 13, 65%) outnumber females (N = 7, 35%). CGD is commonest amongst the Malays (65%) followed by the Chinese (15.0%), Indians (10.0%) and natives of Borneo (10.0%), reflecting the ethnic composition of the country. The mean age of diagnosis was 3.7 years. There was a positive family history in 40% of the cases. Abscess was the main presenting feature in 16 patients (80%) with one involving the brain. Pneumonia occurred in 10 (50%) and one with complicated bronchiectasis. Catalase-positive bacteria were the most commonly isolated pathogen with Chromobacterium violaceum predominating (N = 5, 25%) with consequent high mortality (N = 4, 80%). All CGD patients with C. violaceum infection displayed CD4 + (T helper cells) lymphopenia.ConclusionThis study has shown CGD occurs in the major ethnic groups of Malaysia. To the best of our knowledge, this is the first and the largest series of chronic granulomatous disease in South East Asia which may be reflective of similar clinical pattern in the region. C. violaceum infection is associated with a higher mortality in CGD patients in Malaysia. All the CGD patients with C. violaceum infection in this patient series displayed CD4 + (T helper) lymphopenia. We recorded rare clinical manifestation of CGD viz. brain abscess and bronchiectasis.

Population-specific profiling of CCL3L1 copy number of the three major ethnic groups in Malaysia and the implication on HIV susceptibility.

CC chemokine ligand 3 like-1 (CCL3L1) encodes for CCL3L1 protein, which is a human immunodeficiency virus (HIV) suppressive chemokine and a potent ligand of HIV CCR5 co-receptor. CCL3L1 exhibits variation in the gene copy number (CN) and could influence HIV susceptibility through gene dosage effect. The study aims to determine the distribution of CCL3L1 CN among HIV subjects of Malay, Chinese, and Indian ethnics in Malaysia and to evaluate the impact of CCL3L1 CN on susceptibility to HIV. This study involved 182 HIV patients who attended outpatient clinics of three hospitals in Malaysia and 150 non-HIV (control) subjects. Typing of CCL3L1 CN was conducted via multiplex paralogue ratio tests (PRTs), followed by validation of the CCL3L1 CN by microsatellite analyses. Both Malay and Indian HIV subjects had the CN mode of two, while the CN mode for the Chinese was four. The CCL3L1 gene CN was found to be strongly associated with ethnicity (p<0.001) with the diverse distribution of CCL3L1 CN between the Malay (range=0-6), Chinese (range=0-9), and Indian (range=1-4) ethnic groups. CCL3L1 CN higher than and equal to the average was associated with reduced HIV susceptibility among the Malays (p<0.05). However, the negative results found for the Indian and Chinese need to be further analysed in a larger sample size.

Ethnic Differences in Participation in Medical Check-ups Among the Elderly: Evidence from Malaysia.

There were ethnic differences in the prevalence of non-communicable diseases among the elderly in Malaysia. To examine ethnic differences in participation in medical check-ups among the elderly. A nationally representative data set was employed. Multiple logistic regressions were utilised to examine the relationship between ethnicity and the likelihood of undergoing medical check-ups. The regressions were stratified by age, income, marital status, gender, household location, insurance access and health status. These variables were also controlled for in the regressions (including stratified regressions). The respondents were required to be residents of Malaysia and not be institutionalised. Overall, 30,806 individuals were selected to be interviewed, but only 28,650 were actually interviewed, equivalent to a 93% response rate. Of those, only 2248 were used in the analyses, because 26,402 were others or below aged 60. The dependent variable was participation in a medical check-up. The main independent variables were the three major ethnic groups in Malaysia (Malay, Chinese, Indian). Among the elderly aged 70-79years, Chinese (aOR 1.89; 95% CI 1.28, 2.81) and Indians (aOR 2.39; 95% CI 1.20, 4.74) were more likely to undergo medical check-ups than Malays. Among the elderly with monthly incomes of ≤RM999, Chinese (aOR 1.44; 95% CI 1.12, 1.85) and Indians (aOR 1.50; 95% CI 0.99, 2.28) were more likely to undergo medical check-ups than Malays. Indian males were more likely to undergo medical check-ups than Malay males (aOR 2.32; 95% CI 1.15, 4.67). Chinese with hypercholesterolaemia (aOR 1.45; 95% CI 1.07, 1.98) and hypertension (aOR 1.32; 95% CI 1.02, 1.72) were more likely to undergo medical check-ups than Malays. There were ethnic differences in participation in medical check-ups among the elderly. These ethnic differences varied across age, income, marital status, gender, household location, insurance access and health status.

Copy number variation of CCL3L1 among three major ethnic groups in Malaysia

BackgroundC-C motif Chemokine Ligand 3 Like 1 (CCL3L1) is a multiallelic copy number variable, which plays a crucial role in immunoregulatory and hosts defense through the production of macrophage inflammatory protein (MIP)-1α. Variable range of the CCL3L1 copies from 0 to 14 copies have been documented in several different populations. However, there is still lack of report on the range of CCL3L1 copy number exclusively among Malaysians who are a multi-ethnic population. Thus, this study aims to extensively examine the distribution of CCL3L1 copy number in the three major populations from Malaysia namely Malay, Chinese and Indian. A diploid copy number of CCL3L1 for 393 Malaysians (Malay = 178, Indian = 90, and Chinese = 125) was quantified using Paralogue Ratio Tests (PRTs) and then validated with microsatellites analysis.ResultsTo our knowledge, this is the first report on the CCL3L1 copy number that has been attempted among Malaysians and the Chinese ethnic group exhibits a diverse pattern of CCL3L1 distribution copy number from the Malay and Indian (p < 0.0001). The CCL3L1 ranged from 0 to 8 copies for both the Malay and Indian ethnic groups while 0 to 10 copies for the Chinese ethnic. Consequently, the CCL3L1 copy number among major ethnic groups in the Malaysian population is found to be significantly varied when compared to the European population (p < 0.0001). The mean/median reported for the Malay, Chinese, Indian, and European are 2.759/2.869, 3.453/3.290, 2.437/1.970 and 2.001/1.940 respectively.ConclusionThis study reveals the existence of genetic variation of CCL3L1 in the Malaysian population, and suggests by examining genetic diversity on the ethnicity, and specific geographical region could help in reconstructing human evolutionary history and for the prediction of disease risk related to the CCL3L1 copy number.

T176. INSIGHTS INTO THE ROLE OF ORAL AND GUT MICROBIOME IN THE PATHOGENESIS OF SCHIZOPHRENIA

BackgroundThe role of oral and gut microbiomes in the pathogenesis of schizophrenia has recently come to light with the advancement of sequencing technology. Recent studies have shown that schizophrenia patients have different gut and oral microbiome profiles than healthy people. The interaction of oral and gut microbiota with the brain is facilitated through the gut-brain axis. Several studies have profiled the gut microbiomes in the Malaysian population, however none has investigated its relationship with etiology of schizophrenia. The aim of this preliminary study is to therefore investigate the relationship between oral and gut microbiomes in pathogenesis of schizophrenia in the Malaysian population based on the total transcriptome profiles.MethodsIn this study, 15 schizophrenia patients and 15 healthy people (control) were recruited from the three major ethnic groups in Malaysia, namely Malay, Chinese and Indian. Throat swabs and stool samples were collected and subjected to total RNA extraction. Following this, ribosomal RNA depletion and cDNA conversion were performed prior to sequencing using Illumina platform. Bioinformatic tools will be used to analyse and compare the transcriptome profiles of microbiomes from schizophrenia and healthy subjects. Brain derived neurotrophic factor, serotonin and dopamine assays will be performed using their blood samples too. The results of the bioassays will be then correlated with the transcriptome profiles to identify possible mechanisms by which microbiota could promote schizophreniaResultsThe RNA sequencing results will reveal differences in gene expression profiles between the microbiomes of schizophrenic and healthy subjects. Based on the transcriptome profiles and bioassay results, the possible pathways and mechanisms by which these microbiota can cause schizophrenia will be identified. In addition, candidate biomarkers from the oral and/or gut microbiomes for diagnosis of schizophrenia can be identified.DiscussionThe first and second generations of schizophrenia treatments mostly are based on controlling the symptoms of the disease, whereas treatment based on microbiome dysbiosis correction can tackle the underlying mechanism leading to a precise treatment of the disease.The findings from this study will enable the identification of distinct schizophrenia associated bacteria, that may help in development of new treatments and novel diagnostics for schizophrenia via microbiome-targeted therapy including probiotics and prebiotic.

Outrage in Malaysia: The Politics of Taking Offence

This article develops a general articulation of the politics of offence and outrage by drawing on examples of high profile political furores in Malaysia. In these furores, minority ethno-religious groups and individuals ostensibly caused offence to the majority Muslim Malay population. Although these offences were framed as transgressions of genuine sensitivities, I argue that politics of offence must be seen for the political utility it holds for those who claim to represent the majority group that has been putatively offended. In Malaysia, a key component of this political strategy is the positioning by the United Malays National Organisation (UMNO) of its constituency—the Malays—as being comparatively economically weaker than non-Malay groups. This positioning in turn warrants the continuation of the positive discrimination policies of the New Economic Policy of the 1970s, which was implemented to achieve parity between the major ethnic groups in Malaysia. An example of this was when UMNO vehemently rejected the findings of a think tank that argued that the New Economic Policy’s target of having 30% Malay ownership of equity had been achieved. This positioning of an in-group as weak, and the normalisation of privilege in the Malaysian context, is analysed in view of two other cases studies, Erving Goffman’s concept of ‘the turn’, and Ward Goodenough’s description of the human experience of outrage. I also show that ‘outrage’ as a politico-emotional strategy remains effective despite the momentous results of Malaysia’s general elections in 2018, which might otherwise herald a new era of inter-ethnic co-operation.

Measuring anterior loop length of the inferior alveolar nerve to estimate safe zone in implant planning: A CBCT study in a Malaysian population

Statement of problemThe inferior alveolar nerve (IAN) frequently loops backward before exiting from the mental foramen and spreads several millimeters medially to the foramen. Implant placement in this area may damage the nerve if the anterior loop area is not carefully identified in a radiographic or computed tomography (CT) evaluation. PurposeThe purpose of this observational study was to measure the prevalence of the presence of the anterior loop and to estimate sex and ethnicity-related variations in anterior loop length in the Malaysian population. Material and methodsA total of 100 cone beam computed tomography (CBCT) Digital Imaging and Communications in Medicine (DICOM) files were selected from a pool of 810 ongoing or completed patients in 3 different ethnic groups: Malay (33), Indian (33), and Chinese (34). The DICOM data were imported into commercial software. The IAN was traced with software along with the anterior loop and part of the incisive nerve. The vertical length of the nerve was estimated from the canal to the opening of the mental foramen from the cross-sectional view and translated to the panoramic view. Measurement was made from this point to the most anterior point of the anterior loop by following the trajectory of the nerve and was repeated on the opposite side. A 2-way mixed analysis of variance (ANOVA) test was carried out to evaluate the sex- and ethnicity-related variations (α=.05). ResultsThe anterior loop was present in 94% of the 100 participants. Overall anterior loop length (AnLL) ranged between 0.73 and 7.99 mm with a mean length of 3.69 ±1.75 mm on the left side and 3.85 ±1.73 mm on the right side. Among all participants, no statistically significant differences were found between the left and right sides of the mandible (P=.379). Overall, no significant main effect of ethnicity (P=.869) or sex (P=.576) was found on AnLL measurements. Also, with multiple comparisons, no significant effect was found between each pair of ethnic groups. Men in all 3 ethnic groups had greater AnLL than women. ConclusionsThe anterior loop was present in 94% of the 100 participants among the 3 major ethnic groups of Malaysia. Overall AnLL ranged between 0.73 and 7.99 mm and mean lengths of 3.69 ±1.75 mm on the left side and 3.85 ±1.73 mm on the right side, with no significant ethnicity- or sex-related variations.

Young Women’s Experience of Sexuality: A Battle of Pleasure and Sexual Purity in the Context of Malaysian Society

Little is known regarding the ways in which Malaysian women’s sexuality is constructed, and how their personal experiences, culture, religion and society influence their feeling, attitude and sexual behaviours. This study explored the sexual experiences of young adult women in the developing context of the country. A qualitative phenomenological approach was adopted. Purposive sampling was employed, and a total of 20 participants were recruited, comprised of the major ethnic groups in Malaysia. Four main themes emerged through the phenomenological analysis of the data, including: conditional pleasure; social honour and women’s purity; risk and responsibility; and transcendence of love and emotion. The findings of this study revealed that sexuality was experienced under the control of religion and society with the concern of conserving a woman’s purity and social honour; and imposing risks and responsibility on women. Despite the awareness of women’s right to experience sexual pleasure, it was commonly perceived that sexual activities beyond the social norms will result in social shaming and guilt. The authors recommend appropriate sexuality education regarding sexual assertiveness and communication, safe-sex practices, and improving sexual decision-making skills.

The invited participants represented three major ethnic groups of Malaysia (Malay, Chinese and Indians). An open-ended approach was used to elicit answers from participants. Interview questions were related to participant’s perception towards self-monitoring blood glucose practices, Awareness towards diet management, behaviour to diabetes medication and cues of action.ResultsA total of twenty-one diabetes patients between the ages 35–67 years with physical disability (P1–P21) were interviewed. The cohort of participants was dominated by males (n = 12) and also distribution pattern showed majority of participants were Malay (n = 10), followed by Chinese (n = 7) and rest Indians (n = 4). When the participants were asked in their opinion what was the preferred method of recording blood glucose tests, several participants from low socioeconomic status and either divorced or widowed denied to adapt telemonitoring instead preferred to record manually. There were mixed responses about the barriers to control diet/calories. Even patients with high economic status, middle age 35–50 and diabetes history of 5–10 years were influenced towards alternative treatments.ConclusionsStudy concluded that patients with physical disability required extensive care and effective strategies to control glucose metabolism.Practice implicationThis study explores the patients’ perspectives regarding treatment management with physical disability.

Analysis of CTG repeat length variation in the DMPK gene in the general population and the molecular diagnosis of myotonic dystrophy type 1 in Malaysia

SNP variants associated with non-Hodgkin lymphoma (NHL) correlate with human leukocyte antigen (HLA) class II expression

Large consortia efforts and genome-wide association studies (GWASs) have linked a number of genetic variants within the 6p21 chromosomal region to non-Hodgkin lymphoma (NHL). Complementing these efforts, we genotyped previously reported SNPs in the human leukocyte antigen (HLA) class I (rs6457327) and class II (rs9271100, rs2647012 and rs10484561) regions in a total of 1,145 subjects (567 NHL cases and 578 healthy controls) from two major ethnic groups in Malaysia, the Malays and the Chinese. We identified a NHL-associated (PNHL_add = 0.0008; ORNHL_add = 0.54; 95% CI = 0.37–0.77) and B-cell associated (PBcell_add = 0.0007; ORBcell_add = 0.51; 95% CI = 0.35–0.76) SNP rs2647012 in the Malaysian Malays. In silico cis-eQTL analysis of rs2647012 suggests potential regulatory function of nearby HLA class II molecules. Minor allele rs2647012-T is linked to higher expression of HLA-DQB1, rendering a protective effect to NHL risk. Our findings suggest that the HLA class II region plays an important role in NHL etiology.

Gauging Malaysian Consumers Perspectives about Corporate Social Responsibility

This research was carried out on the premise that as CSR was becoming popular among corporations in Malaysia, it was important to gauge consumers? beliefs and perspectives about CSR and the CSR issues that mattered to them most when purchasing a product.? The research covered 12 states of West Malaysia targeting 120 respondents each, bringing the total sample size to 1440 respondents that were selected via quota sampling method for representation of major ethnic groups in Malaysia. Respondents were presented with three main questions to gauge their perspectives and were allowed to tick as many responses as they wished from a list of options available. Basic descriptive statistics were deployed to interpret the findings of the study. The findings give evidence that Malaysian consumers are becoming highly aware about CSR and the type of CSR activities organizations should undertake. The research also seemed to imply that consumers in Malaysia have begun taking into account an organization?s CSR involvements when making a purchase decision.
